What does internally and externally mean when referring to a synopsis of a novel?

Example: What, if any, external events are keeping them apart as a couple? What internally (this is a must) is keeping them from being together as a couple?

In writing a Romance novel, these are questions the Arthur needs to ask himself and to put the answer to these questions in the synopsis before sending it to the editor/publisher, etc.

What does internally and externally mean in this case? Someone will probably say in the book and out of the book. If that’s it, what do they mean by out of the book? See…I’m confused! Lol

I need an answer written please as if it were a book like : How to read the book dummies for dummies or math for morons, etc. lol

Please help explain this in simple and easy terms but not too retarded like (even though that is maybe what I actually need) lol

Internal conflicts are the conflicts that occur within each character that keeps the hero and heroine apart. They're individual, personal reasons why they cannot be together. (such as she's had a bad marriage and is afraid to do it again).

External conflicts are things outside of the character that keep the couple apart. such as a meddling mother in law. Or a bomb that threatens her life.
